Please print out one of the packet’s below, fill it out as best as you can, and bring it to your appointment. If the packet is not completed, we may have to reschedule your appointment for a different day and time.
If you feel you need help in filling the packet out, please come to your appointment at least 30 minutes early, or if your appointment is at 8 AM, please call (209) 725-7900 before 5 PM the day before to let staff know you may need help filling out the packet. For a restraining order against an intimate partner/former intimate partner, spouse/ex-spouse, or other parent of your child, please use this Domestic Violence TRO Packet.
